I politely ask my readers to please indulge me while I have this private rant.
You see, every day I go to work within a challenging healthcare system while my Congressional representative plays games on Capitol Hill.
The good congresswoman to whom I elected appears to be sitting on the fence. Yes ma’am, I’m talking about you. I warned you that if you did not answer my e-mail then you WOULD read about it in my blog. Yes, I saw you in the TV commercial espousing “working to make a better healthcare system” and noted the sponsor: a pharmaceutical “foundation.” Pulleease. You were the first elected Democrat within my district since Noah launched his Ark and you have yet to commit an opinion regarding the public health option. Obviously you had an agenda when you entered public office and it did not include the constituents of your district.
What is it with Florida politicians that make you so ethically-challenged? The debacle of the 2000 election. Instant replay in 2004. Representative Mark Foley and his predilection for White House pages. Tom Feeney and the Abramoff scandal. And on and on and on. Is it something in the water?
They say the worst thing that anyone could ever witness is the making of political decisions and the way that sausages are made.
Well, please pass the sausage. I’ll take them over politics any day.

Mother Becomes Pregnant With the Wrong Baby! The Ultimate Nightmare
The MSNBC story, Wrong-Embryo Baby’s Parents Laud Guardian brings to mind a line from Tina Fey’s movie, Baby Mama: “Life is messy.” Fey plays the character of a thirty-seven year old woman who was informed that she only has a million-in-one chance of conceiving because of an abnormal uterus. She hires a surrogate who unknowingly is not pregnant with Fey’s child but her own. When the surrogate discovers the error, she must break the news to Frey who is of course, devastated. Art imitates life.
Carolyn Savage was a 40 y.o. woman who received in Vitro-Fertilization and on the third attempt, successfully conceived a baby. Unfortunately, it was the wrong baby; a nightmare no one should ever experience. Savage was carrying a baby who belonged to the Morrell family who had frozen embryos at the same infertility clinic.
As tragic as this may appear, all parties involved rose to the highest level of integrity regarding correcting this unfortunate error. The clinic informed Savage immediately upon discovering their mistake. As a physician who has witnessed my share of ethically-challenged administrators and colleagues in the medical profession, it took a tremendous amount of courage to claim ownership to an error of that magnitude. Savage was given the option of terminating the pregnancy or giving the baby to its biological parent. The Morrells had not planned to have a baby anytime soon but were informed that another woman was carrying theirs. What a moral dilemma.
As a former infertility patient and physician who became a mother after age 50 through the miracle of adoption, I truly felt their pain. Last week Savage delivered a healthy baby boy and gave it to the Morrells. There are no words that can articulate Savage’s valiant act.
To err is human but the effects are still shattering. May this level of incompetence never be allowed to recur again.

Arkansas Woman Pregnant with Two Babies… Not Twins!
One of the most miraculous chapters of life is the act of conception and the mysterious factors of birth; such is the case of Mrs. Julia Grovenburg who happens to be pregnant with two babies that were conceived at different times within her menstrual cycle. While twins occur in 3. 2% of 4 million annual U.S. births there have only been 10 reported cases of a women becoming pregnant who was ALREADY pregnant. According to Good Morning America Grovenburg received the shock of her life when a routine ultrasound revealed that another smaller fetus was growing inside of her.
The condition called superfetation has been seen primarily in horses, but is extremely rare in humans. So rare, in fact, that I could not find mention of this condition in the American College of Obstetricians and Gynecologists search engine. This condition poses several possible health related conditions; and what’s even more ironic is that if the babies are born on their due dates they will be born in entirely two different years!
These are the types of occurrences that keep physicians humble and there is much to be learned from her case. I will continue to observe Mrs. Grovenburg’s pregnancy.

What Every Woman Should Know About Dollar Store Ovulation Kits!
So here we go again! First a dollar pregnancy test and now an ovulation test for the same great price ($1.00)

After the tremendous positive response to my earlier post regarding the $1.00 pregnancy test, I received a phone call from SCI’s Vice President, Mr. Abedi. He was delighted that the blog was well received and I in turn was delighted with his product. He explained that the company’s only makes a few cents above its production cost but that they deal in volume. It was so refreshing to hear about a company that wasn’t going to bankrupt its customers in order to make a profit. Abedi continued to describe the company’s array of products including an ovulation kit for $1.00. You can only imagine how far my jaw dropped. As a former infertility patient, I am well-versed with ovulation kits and could not believe they were being sold for . . . a dollar? Abedi quickly added that the ovulation kits were only available at the Dollar Tree store. So, off I went to my local Dollar Tree store to see for myself.
Upon my arrival and to my utter delight, both pregnancy and ovulation tests were readily available. The pregnancy tests were behind the counter while the ovulation kits were displayed up front. The cashier must have read my mind and offered the following explanation: The pregnancy tests are a popular item among teens who attempt to steal them. And the ovulation kits are displayed in front of the cash register because “teens are not trying to get pregnant.” It made perfect sense. And just like their pregnancy test counterpart, the ovulation tests are 99% accurate.
Quality and at affordable prices. Who would want to pay more?

The Deeper Meaning of Jay-Z and Some Thoughts For Pregnant Women

I’m not a great fan of Hip Hop, but I do appreciate the enchantment of the spoken word. Of late, there’s been a lot of discussion regarding Hip Hop artist, Jay-Z’s latest CD and his interview in Oprah Winfrey’s O Magazine. My interest in Jay-Z does not lie in his musical talents, but rather his humble beginnings.
What Jay-Z and I share in common is our former domicile in the Marcy Housing Projects in Brooklyn (although my family relocated the year he was born). Living in a housing project is a transformative experience. What doesn’t kill you makes you stronger. Although my early childhood was spent in a small Long Island town called Amityville, I always acknowledge Brooklyn first. Perhaps because of its sense of community and my surrogate mothers who made me tow the line. Heroin addiction was rampant during my tenure in the Marcy. For Jay-Z’s generation, it was probably crack-cocaine. Yet, through the grace of God and the watchful eyes of my surrogate family, I was not directly affected. Jay-Z was one of the lucky ones. Most men that I grew up with in the Marcy were dead before the age of thirty.
Brooklyn is unique because of its creativity. I can’t offer an explanation. It simply is. Mary Tyler Moore. Barbara Streisand. Judge Judy. Rosie Perez. Spike Lee. Larry King. Chris Rock. Lauren Bacall. I think you get my point. Each one of these celebrities possessed a gift that I’m sure their pregnant moms never dreamed they would have.
I’ve been an obstetrician for almost twenty-two years yet I’m still in awe at the miracle of pregnancy. We never know who is coming but are ecstatic when they finally arrive. Within humble wombs lies the promise of infinite possibilities. Jay-Z was just one of them and they’ll be many more to come.

The REAL truth about “Morning Sickness” and how you can get rid of it!
In my quest to help “moms-2-b”, a recurring question emerges: “How do I get rid of morning sickness?” Although the Internet is rampant with well intended but “incorrect” replies, the correct answers have finally arrived.
So, you’re retching, gagging, vomiting and plain miserable. Sound familiar? Well, you’re not alone. Approximately 50 to 90 percent of pregnant women have experienced these symptoms at least once during their pregnancy but believe it or not, it eventually stops. Listed below are some strategies for giving you some relief.
Step one:
•Avoid triggers such as spicy foods, fried food, and offensive odors
•Eat frequent light meals high in carbohydrates and low in fats. Also, try to avoid having a completely empty stomach. The stomach produces acid and when empty aggravates the problem.
•Eat dry toast or crackers before getting out of bed in the morning
•Try the BRATT Diet (bananas, rice, applesauce, toast and tea)
•Cold fluids are tolerated better than hot
No relief? Try Step Two:
•Medical studies show that ginger is quite effective. Ginger tea, candy and lollipops might alleviate the problem. Some of my patients have used ginger with good results.
•Vitamin B6 25 mg three times a day has been helpful for nausea but not as effective for vomiting
•Take prenatal vitamins at night before going to bed
Still miserable? Proceed to Step Three. Ask your OB practitioner for drugs (the legal ones of course).
Here are the ones most commonly prescribed:
•Phenergan® or Promethazine is usually the first line of defense. It may be given orally although most physicians prescribe the rectal suppositories if the patient cannot tolerate oral food or medication
•Reglan® or Metoclopramide helps empty the stomach quickly and is taken every eight hours, but only as needed
•Zofran ODT ® or Ondansetron is a rising star. It is usually given to patients receiving chemotherapy but in the past five years has become very popular in treatment of morning sickness. It provides the greatest relief for most of my patients but it’s not cheap. However, if you shop around, you can usually find bargains with the generic brand.
Remember that most symptoms usually subside by the middle of your second trimester so try to hang in there.

SMART Tips for Taking a Hospital Labor and Delivery Tour
One of the most magnificent moments in a woman’s life occurs in a hospital delivery room, yet most women do not take advantage of viewing the location in advance. Many hospitals offer free tours of their labor and delivery suites, and expectant families should take full advantage of this opportunity. Most labor and delivery suites look like master bedrooms and offer LDRs (labor, delivery and recovery rooms) equipped with a birthing bed, so mothers can remain in one room during their entire hospital experience. Here’s what to do!
S = Seek information regarding the nurse-to-patient ratio. The ideal situation is one nurse managing one laboring patient; however, a labor room nurse should never be responsible for more than two patients at one time.
M = Make certain that you visit the LDRs or birthing rooms, the operating room, the hospital registration office and the nursery. Also inquire about fetal monitors (machines that measure babies’ heartbeats and mothers’ contractions). Do they have alarm systems that alert hospital staff when a baby’s heartbeat becomes abnormal?
A = Ask what kinds of perks the hospital offers, such as a special entrance for mothers in labor, special parking areas or valet parking, rocking chairs or birthing balls in the LDR, or access to high-risk specialists in the hospital. Does it have a level 3 nursery or 24-hour anesthesia coverage? What is the visitor policy?
R = Request to see a demonstration of the nursery security policy. Most nurseries have very detailed security measures to ensure the safety of newborns. Does the hospital have a cyber-nursery with infants’ pictures on the hospital Web site? Does it offer a newborn channel with up-to-date movies on caring for newborns?
T = Take time to speak with nurses from both labor and delivery and the nursery to ask questions. In the event of unforeseen emergencies, it is always helpful to have a pediatrician readily available as opposed to having to locate one at the time of delivery.
An advance tour of the labor room not only removes the mystery of the unknown but provides a firsthand view of where your baby will make his or her grand entrance.

Who Else Wants Affordable Healthcare?
Did you see it? The makings of history? Oh what a joyful sight! After sixty years of several failed attempts, maybe we’ll finally get it right. It took the courage of a leader to venture down the road less traveled. Despite the hate, propaganda and vitriolic behavior, President Obama stood firm, like many of us prayed he would.
Obama delivered on his promise: a public health option. The party is over for the insurance company monopolies and I’m not shedding a single tear.
The rights of passage I endured during my eight years of training were never meant to be used for the benefit of greed. It was for the exclusive benefit of my patients. I often lament on how physicians arrived at this ungodly juncture and to a certain extent, it’s our own fault. Most of us were too afraid to organize for fear of being in violation of anti-trust. Of losing precious hospital privileges. Of being disenrolled from insurance plans. Of being labeled a “trouble maker.” So instead, we did nothing and the unscrupulous took over.
Some of us threw in the towel and retired. Others added “spa boutiques” to compensate for shrinking revenue. The desperate (and greedy) ones added additional volume to their office schedule and almost killed patients in the process. Some whined about “Obamacare” in the “physician-only” chat rooms and called him every name in the book. But deep within the recesses of their hearts, they knew they were witnessing a hero. Our profession was a bumbling mess, yet no one had the courage to fix it. It was the nurses (God bless them) that stormed the senate hearings in protest so that our voices could finally be heard.
When the smoke clears and the dust finally settles it is the patient-physician relationship that must be held sacred. Med school taught us the standards, the ethics and the integrity of our profession. Let us NEVER allow it to be compromised again.

The Secrets of Pregnancy
Have you ever wondered what was really going on in there? Well you are in luck because today I will tell you all about it!
Over the next nine months, your body changes can leave you baffled: Should you call a doctor? What’s normal? Below is a roadmap to 10 possible pregnancy side effects and the best responses to them:
1. Vaginal Spotting
Some women don’t know exactly when they became pregnant because they had “two periods” in one month. While any vaginal bleeding during pregnancy warrants a doctor’s visit, the cause is usually the result of the fertilized egg settling into the uterus.
If you have a positive pregnancy test and complain of bleeding (heavier than what you have in the beginning or end of your period), your doctor should order an ultrasound test – which uses sound waves to allow the doctor to see the fetus – to make certain it is in your uterus and not your fallopian tubes (the tubes through which a mother’s egg travels to reach her uterus).
If you’re pregnant and the bleeding or spotting is accompanied by abdominal pain or cramping, see your doctor – it could be a symptom of an underlying problem.
2. Dull Abdominal Pain
As your baby grows, uterine and abdominal muscles and ligaments stretch, particularly around the 20th week. The ligaments help the uterus attach to the wall within your pelvis. The sensation may feel like a stretching rubber band, causing a dull pain (called round ligament pain) that starts in the center of your lower abdomen and moves toward your hips.
This occurs more often in first pregnancies and you can treat it by placing a heating pad, set on low, on your stomach. The warmth helps relax the muscles and dulls the pain. The discomfort usually disappears as the baby grows and the ligaments get used to stretching. Speaking of stretching, the third trimester usually brings on another side effect of pregnancy – stretch marks.
3. Teeth Problems
Toothaches and swollen, bleeding gums commonly develop when you’re expectant. Here’s why: Your baby requires a lot of calcium for his or her developing bones, and whatever calcium you get from food or drinks goes to baby first, leaving you short. (You may need to take calcium supplements.)
4. Skin Discolorations
Not everyone gets that expectant-mother’s glow. In fact, up to 70% of women develop darkened skin. Called chloasma or “mask of pregnancy,” the facial stains are usually due to genes or hormonal changes.
Sunscreen and limiting your sun exposure may prevent the discoloration. Your skin probably will clear after delivery, but if it doesn’t, see a dermatologist. Also, don’t use skin lighteners or vitamin A while you’re pregnant: Vitamin A has the potential for causing birth defects and skin lighteners may contain bleach.
You also may develop a dark line down the middle of your abdomen (called the linea nigra) and your nipples may darken. Both stem from increased estrogen and melatonin hormones. The areas usually lighten after the delivery, but sometimes they never disappear completely.
5. Thicker (Then Thinning) Hair
One happy side effect is a lustrous mane. Alas, it’s short-lived… and misleading. Your hair isn’t actually growing more – you’re just shedding less.
During pregnancy, your hair’s “growing stage” increases, meaning fewer hairs fall out. After delivery, however, the “resting stage” (when hair stops growing) is longer than usual, and hair loss increases.
It all should revert to normal within 15 months of delivery, but if hair loss is excessive, see a dermatologist.
6. Leg Cramps
Pregnant women sometimes complain about leg pain, especially in their calves. It may mean something simple, like you’re not drinking enough water or getting enough calcium.
But it could be something more serious, such as a blood clot. They are usually swollen, red and painful to touch. If you’re concerned, contact your physician or health-care provider to order a test (the Doppler flow study) that measures the blood flow through the leg.
If there’s a blood clot, a doctor will immediately give you blood thinners and monitor you weekly.
7. Buttock Pain
Pregnancy can be a literal pain in the butt for some. If the baby’s pressing on a nerve near the spine, you may develop sciatica, which causes a shooting, burning sensation in the buttocks, a pins-and-needles sensation or numbness. Pain in the lower back pain, back of the thigh or that runs down the back of the leg and foot are also symptoms.
You’ll be uncomfortable, and a doctor can do little to change the baby’s position in your womb. Apply a heating pad set on low to the aching spot for temporary relief.
8. Vaginal Itching
Annoying yet common, vaginal itching results from an increase in fungus in your body.
Here’s why: Your immune system is less active during pregnancy, fighting fewer “foreign objects” and leaving fungus free to frolic and cause yeast infections. With a more vigilant immune system, your body could, in theory, reject your baby as an intruder.
Diet also raises the risk of yeast infections, especially dairy products, such as ice cream, yogurt, cheese and milk. Of course, getting enough calcium is crucial for your developing baby, so look for non-dairy, calcium-rich foods, such as spinach, beans including soybeans, fortified cereals and canned salmon with the bones.
9. Varicose Vaginal Veins
Also known as vaginal varicosities, these are worm-looking veins that show up in the last 12 weeks, mostly after week 35 of your pregnancy. That’s when the baby’s head is snug in your pelvis, pressing against the vaginal veins and enlarging them.
They’re ugly but usually pose no threat. Some midwives recommend placing 1/4 cup of Epsom salts on a moist washcloth, mixing until it’s the consistency of paste, then placing it on the veins (using light pressure) for 10-15 minutes. The salt will stimulate circulation and help relieve discomfort. Do not wear Spandex garments – it may constrict the vagina and your growing abdomen.
10. Baby’s Hiccups
In the last five weeks, you might notice your belly jumping up and down rhythmically. Relax – nothing’s wrong with the baby – he or she is just practicing how to breathe.
The muscle that helps humans breathe – the diaphragm – is getting stronger. Until your baby is born, he or she will receive oxygen through the umbilical cord. But once the cord is cut, the baby has to breathe on his or her own, so think of those hiccups as a dress rehearsal for breathing.
Pregnancy is both an adventure and a walk in faith. As your body transforms, relax. Know that rarely will any condition occur that you and your medical team can’t handle.
